Billing Statements
If you receive a statement after your visit and have questions about your balance, please follow the contact information listed on your statement to reach our billing department. They have access to detailed account information and can provide the most accurate explanation.
Referrals and Prior Authorizations
Some insurance plans require referrals or prior authorizations—even for established patients. These requirements are set by insurance companies and may change from year to year.
Patients are responsible for ensuring that required referrals are in place and match the reason for the visit before the appointment.
Appointments and Scheduling
If you need to cancel or reschedule an appointment, please let us know as soon as possible. Early notice allows us to offer that appointment time to another patient who may be waiting to be seen.
Medical Records
For privacy and accuracy, medical records requests must be submitted in writing using our Request for Medical Records form. This helps us protect your information and process requests efficiently.
