New Patient Paperwork

Thank you for choosing Shoal Creek Foot & Ankle Center. To help us get you checked in quickly, please download and complete the New Patient Registration Packet before your visit. Bring the completed forms to your first appointment so we can process your information efficiently.

Your First Appointment

To keep your first visit running smoothly, please arrive on time and complete any remaining paperwork. Bring the following items with you:

  • Insurance card(s) and a photo ID
  • Your medical and surgical history
  • A list of current medications, including dose and frequency (prescription and over-the-counter)
  • Recent test results, MRI/CT discs, and other relevant medical records

Footwear: If your appointment involves pain while standing or walking, bring the shoes you wear most often.

Minors: Patients under 18 must be accompanied by a parent or legal guardian.

What to Expect at Your First Appointment:

  1. Check-In: Stop at the front window when you arrive. Our reception team will provide your New Patient Registration Packet if you haven’t completed it beforehand. While you finish your forms, we will verify your insurance.
  2. Payment: After your paperwork is complete and insurance is confirmed, we’ll collect any required payment based on your coverage. For most patients, this is a specialist co-pay.
  3. Exam Room: You’ll be taken to an exam room, where you can remove your shoes and socks and get settled into the exam chair.
  4. Medical Assistant: A Medical Assistant will take your vitals, review your medical history, go over your symptoms, and take x-rays if needed.
  5. Doctor’s Visit: Once that is complete, the doctor will perform the exam, explain your condition, and review your treatment options.

Payments

  • With Insurance: New patients pay their co-pay (if required) at check-in. After your insurance processes the claim, you are responsible for any remaining balance such as deductibles, coinsurance, or non-covered charges. Patients with Medicare or VA healthcare follow the same process.
  • Without Insurance: A new patient visit is $150. Additional charges may apply for procedures, tests, or supplies such as ingrown toenail treatments, x-rays, or night splints. Call us if you’d like an estimate in advance.

Payment is due at check-in. We accept cash, checks, American Express, Discover, MasterCard, and Visa.

Insurance

Shoal Creek Foot & Ankle Center accepts most major insurance plans, including Aetna, Anthem, Benefit Management, Blue Cross Blue Shield, Cigna, Cox Health Plans, Humana, Medicare, TRICARE, United Healthcare, and UMR.

Coverage varies by plan. Patients should confirm directly with their insurance provider that our clinic and physicians are in-network for their specific policy.

Late Arrival & Missed Appointments

To keep the schedule running smoothly, patients who arrive more than 15 minutes late may be asked to reschedule.

If you can’t make your appointment or know you’ll be running late, please contact us as soon as possible. Appointments that are missed or canceled with less than 24 hours’ notice may be subject to a $35 fee.

Request for Medical Records

To request your medical records, submit a completed Request for Medical Records form. If the patient is under 18, the authorization must be signed by a parent or legal guardian. Completed forms may be returned in person, electronically, or by mail.

Please allow up to 30 business days for processing. If any fees apply, we will notify you before the records are released.
