New Patient Paperwork
Welcome to Shoal Creek Foot & Ankle Center. We value your time—and ours—so we ask new patients to complete their registration paperwork before their first visit whenever possible.
Completing your forms in advance helps us:
- Check you in efficiently
- Verify your insurance accurately
- Spend more time focused on your care
Please download and complete the New Patient Registration Packet and bring it with you to your first appointment.
Your First Appointment
To keep your visit running smoothly, please arrive on time and bring the following:
- Your insurance card(s) and a photo ID
- Your medical and surgical history
- A list of current medications (including dose and frequency, prescription and over-the-counter)
- Recent test results, MRI or CT discs, and relevant medical records
- Footwear: If your visit involves pain while standing or walking, bring the shoes you wear most often
- Minors: Patients under 18 must be accompanied by a parent or legal guardian
Being prepared helps your provider make the most informed decisions about your care.
What to Expect at Your Visit
- Check-In. Stop at the front window when you arrive. If you haven’t completed your paperwork, our reception team will provide it at check-in. While forms are being completed, we’ll verify your insurance.
- Payment. Once paperwork and insurance verification are complete, we’ll collect any required patient payment based on your coverage. For most patients, this is a specialist co-pay.
- Exam Room. You’ll be taken to an exam room, where you can remove your shoes and socks and get settled.
- Medical Assistant. A medical assistant will review your medical history, take vitals, discuss your symptoms, and obtain X-rays if needed.
- Doctor Visit. Your provider will evaluate your condition, explain findings, and discuss appropriate treatment options.
Payments
With Insurance. New patients are responsible for their co-pay at check-in, if required by their plan. After your insurance processes the claim, you are responsible for any remaining balance, including deductibles, coinsurance, or non-covered services. Patients with Medicare or VA healthcare follow the same process.
Without Insurance (Self-Pay). The self-pay fee for a new patient visit is $150. Additional charges may apply for procedures, tests, or supplies (such as ingrown toenail treatment, X-rays, or night splints). If you would like an estimate in advance, please contact our office.
Payment is due at check-in. We accept cash, checks, Discover, Mastercard, and Visa.
Insurance
Shoal Creek Foot & Ankle Center accepts most major insurance plans, including: Aetna, Anthem, Benefit Management, Blue Cross Blue Shield, Cigna, Cox Health Plans, Humana, Medicare, TRICARE, UnitedHealthcare, and UMR.
Insurance coverage varies by plan. It is the patient’s responsibility to confirm that our clinic and providers are in-network for their specific policy.
Late Arrival & Missed Appointments
We schedule appointments carefully to respect both patient and provider time.
- Appointments missed or canceled with less than 24 hours’ notice may result in a $35 fee.
- Patients who arrive more than 15 minutes late may be asked to reschedule.
- If you need to cancel or are running late, please contact us as soon as possible.
Request for Medical Records
Patients may request copies of their medical records by submitting a completed Request for Medical Records form. For patients under 18, the request must be signed by a parent or legal guardian.
Completed forms may be returned:
- By mail
- In person
- Electronically
Please allow up to 30 business days for processing. If fees apply, you will be notified before records are released.
