Please be advised that Dr. Brittany M. Ciaramello, DPM, AACFAS, has accepted a position with a practice in another state. May 15th, 2022, will be her last day at Shoal Creek Foot & Ankle Center. We wish her the absolute best in her new venture.
Dr. Shelly D. Sedberry, DPM, AACFAS, founder of Shoal Creek Foot & Ankle Center, along with our entire healthcare team, will continue to provide the same quality healthcare you received from Dr. Ciaramello. If you wish to remain a patient of this practice, you do not have to do anything.
If, however, you would prefer to seek medical care elsewhere, you are free to do so. You may pick up a copy of your medical records at our office or you may ask us to transfer them directly to another office. For either of these options, we will need a signed request form to transfer records. You may find the form on our website at www.shoalcreekfac.com. You may return the form to our office in person, electronically, or by mail.
